MS Word (92Kb) or PDF (42kb) Please try and make sure that the walker fills in ALL the questions. Sections A&B are compulsory and must be signed. Section C is not so important, but we would like it all filled in. Please state that they MUST let us know if any of the information changes, that the information will only be kept for GGHW records and will not be shared with any other organisation. It is important that a new walker fills in the OHQ before the start of the walk to ensure that they are covered my the insurance AND that we can enter their first walk in the database. Without the OHQ their name will not appear in the tick list. All OHQ's and Walk Registers to be sent to Ann Walsh immediately following the walk.
This is for major incidents only. Cut fingers etc do not
require a form. 2. Leaders Contact NumbersView Leaders contact details Password protected - read only. Please mail in your mobile number and any corrections to your details
Now that we have the National Database, and
we find it impossible to keep up to date with the details, it has been decided
not to include notes. 4. Risk Assessments and MapsAll routes are posted below. Leaders are expected to report any changes to risk on any route so that the assessments are always up to date. Failure to do so could affect our insurance. Please read the Generic Risk Assessment and be aware of the content. There is also an example in the on-line Training Manual on the WfH web site. Please note that all risk assessments that cover the viewpoint at the top of The Coomb woods, or the track across the Holies to the main road atop Streatley Hill now forbid the use of the steep stepped paths that descend from the viewpoint to the Youth Hostel, or from the gate in the dip to the top of The Coombe. If a walker decides to take a short cut up or down either path and leave the main group, they MUST be informed that they are no longer covered by our insurance. Similarly, any leader that leads a walk up or down these routes will not be covered. If you walk a route that is
not included below, please submit a full
Risk Assessment form for the new
route. You can view, or download a current assessment for the routes underlined.
The remainder are in the process of being updated. Under the Ordnance Survey License, leaders may download, print and use the maps solely to aid them in leading a Health Walk. A/B routes:
